SUBSTANCE ABUSE ASSESSMENT WITH COURT REPORT
If you are in need of a drug and alcohol assessment that DOES NOT pertain to driver's license revocation, we provide substance abuse assessments with court reports which have been prompted for legal purposes (probation, family court, Department of Human Service, divorce, and other court related purposes).  This type assessment can be tailored to meet the specific needs of the client and the agency or entity that is requiring the assessment to be completed. This assessment may also be used for cases such as Minor in Possession, Public Intoxication and to fulfill school requirements.
Cost for a Substance Abuse Assessment not needed for license reinstatement is $180. We do accept all major credit and debit cards for your convenience. Payment is due to begin services.
COMPREHENSIVE MENTAL HEALTH ASSESSMENT WITH REPORT
We also provide mental health evaluations that are often used in legal settings and for purposes mentioned above. These are comprehensive evaluations that include both mental health and substance abuse assessment, but cannot be used for driver's license reinstatement.

Important information about this type of assessment:
  • We strongly believe that while this type of visit may be prompted by involvement by the legal system, this remains your service which you are voluntarily a participant in.
  • We will not share the report or other information derived from the assessment with any entity that you do not provide us written permission to release it to.
  • We believe in full transparency and will not withhold your access to your report
  • You will maintain authority as to how, and to whom, this report is shared.
  • Approximate length of office visit for this type of assessment is 1.5 hours and often requires one (1) follow-up appointment.
  • While you will generally not leave with this report at the end of this session, you will be provided an estimate when this will be ready at the end of your assessment (estimate provided after the final appointment if multiple appointments are required). We do offer an expedited service which may require an additional fee, please inquire about this with your provider.
  • Insurance is not accepted for this service, and payment is due at the start of the appointment.
  • After signing a release of confidential information, we will also be able to forward a copy of the court report to your attorney, probation/parole officer, etc.

*Due to the individualized nature of these type of assessments, this assessment may take more than one session to complete. The first session will be scheduled for 90 minutes and subsequent sessions will be determined and scheduled at the end of the initial session. The cost for this services includes all necessary sessions and therapists time writing the assessment report for the individual.
